The Healthcare Integrator is a Registered Nurse who works directly for the Chief of Medical Staff who leads all Medical Management initiatives. Medical Management broadly defined means to establish programs or systems that improve clinical outcomes and manage rising healthcare costs. The three major components of our Medical Management program are our Case Management, Disease Management and Utilization management services. Our Medical Management team, in collaboration with the SGH and primary care managers, facilitate staff education, awareness and use of approved clinical practice guidelines, the standardization of care for defined patient populations and care coordination for complex patient care issues.
